I created this simple 2 page self reflection, suitable from junior infants to 1st class.
All questions require colouring a smiley face, thumbs up/middle/thumbs down, or circling a visual.
Questions/statements include:
How I feel about coming to school
How I feel about my teacher
How I feel about talking in front of the class
I can talk to my teacher if I have a problem or worry
I try my best at school
I have a best friend
I need help with...
I am good at...
etc.
For the younger children, I called out each section and had them complete it as I read. e.g 'The red box says... How I feel about coming to school- if you feel upset coming to school, colour the crying face, etc'. They found this very straightforward and the results were very informative for me, particularly for their emotional and social development. Parents were really delighted with this work sample during the parent teacher meetings, and found it great to see how their child perceives their own school life. I have included a word doc if you want to edit any items but the formatting may be disrupted. I have also included the PDF doc, if you are happy with the content.
