School Support Plus Plan - SSP+ plan example - Level 3 Continuum of Support Yearly SET Plan
This is an example of a completed School Support Plus plan which is a long term yearly support plan for pupils who receive support at the highest level (level 3) of the continuum of support. This plan follows the NEPS template and will serve as a useful example for teachers who are new to SET. Following the introduction of the new SET allocation model in 2017, this "School Support File" is the recommended long term support planning template for schools to use.
Also included are samples of the completed basic needs and learning environment checklists AND a clear, easy to follow and detailed guide on how to complete a support plan. A copy of the blank templates are provided as well.
I am the special needs coordinator in a large school which has well over 50 pupils on the SSP+ level of support. I have taught in support for a number of years and have an undergraduate degree in psychology. I have written and reviewed hundreds of Support Plans for students with a wide variety of learning needs.
This sample School Support Plan is based on real targets that I have worked on with students. The name of the child in the sample SSP is fictional and details of professional reports and about the child have all been changed or fabricated as they are for illustrative purposes only.
In order to demonstrate how to complete the plan in full, I have completed the 'Support Review Record' section. In reality, this sections of the plan is left until the end of the year to complete.
