Classroom Support Plan/ CSP - Level 1 Continuum of Support - Student Support File Format
This is an example of how to complete a Classroom Support Plan, which is level 1 of support using the Continuum of Support framework. Also included in this download is a comprehensive guide on how to complete a support plan and to identifying suitable targets.
The purpose of this document, is to illustrate how to complete the plan what can be included in each section only. Support plans are highly specific to each child and their individual needs and so this plan is not suitable to use for another child.

I have taught as a class teacher, a SET, and an autism class teacher for over a decade and have an undergraduate degree in psychology. I have also been the special needs coordinator in a large school with very high levels of SEN. I have written and reviewed hundreds of Support Plans (at classroom support, school support, and school support+ levels) for students with a wide variety of learning needs.
This sample Classroom Support Plan is based on real targets that I have worked on with students. The name of the child in the sample SSP is fictional and details of professional reports and about the child have all been fabricated as they are for illustrative purposes only.
