I have created a short learning survey for pupils to complete to get to know them a bit better. I generally complete it in September and again in January. It is a handy reference for PT meetings and end of year reports.
There are two PDF versions of the Learning Survey contained within this purchase (see images) and on the second page there are multiple choice questions to investigate the children's own perception of their own skills and learning.
